Casual timesheet – this Word document should be completed and submitted to enquiries@horshamsportsservices.co.uk at the end of every month that work has been undertaken in. The form should be signed and dated before submission. The work claimed for will be paid by bank transfer for by the 20th day of the following month (eg work undertaken in the month of February will be paid for by 20th March).

Written Statement of Employment Particulars – to relay the terms and conditions of the particular role being recruited for
Pension letter – to detail options linked to joining the government-backed NEST workplace pension scheme
Company Policies – once recruitment is complete you will be issued with a password in order to access the company’s policy documents online on the staff section of the HSS website
Application Form – detailing educational and professional qualifications, experience, personal statement and interests
Equal Opportunities Form – supporting Horsham Sports Services’ commitment to equal opportunities in its recruitment and working practices
Self Disclosure Form – supporting Horsham Sports Services’ commitment to the safeguarding of children and vulnerable adults

PERSONAL DOCUMENTS THAT MAY BE REQUIRED FOR CHECKING (AS ADVISED BY THE HSS RECRUITING STAFF MEMBER)
Passport – linked to checking a person’s right to work in the UK
Coaching qualification certificates – eg Football Level 1
Other relevant training certificates – particularly your most recent first aid qualification and safeguarding training
Existing Disclosure and Barring Service (DBS) certificate – if you hold one of these linked to another role you undertake
DBS supporting documents – if HSS requires to order a new DBS for you through its provider there are a wide range of personal documents can be used to support an online application. The most common ones used are: passport, driving licence, birth certificate and a utility bill or bank statement dated less than 3 months old to prove address
